Who is the Forum Moderator? Is this guy Dan, Tom, both of them, Deadron, or someone completely different?

"Forum Moderator" is a catch-all title. Any moderator is given the option to post using that key. This is so that warnings and such will have a more official appearance.

Not exactly. Moderators are still their own person, internally -- they're just displayed as Forum Moderator when they make a post in an official capacity.
